Give to McCabe at Ward 13 to craft the Knight Guard weapon mod.

Obtain by equipping the Crown of the Red Prince, then entering the mausoleum in the Pathway of the Fallen on Losomn to reveal the secret chamber where you'll find the Cremated Soul Ash.
